Last week, we showed you how to choose a provider and make an appointment. This post will help you prepare for your appointment.
If it’s your first time visiting a provider or if you have a new health insurance plan, there are a few things you’ll need to make sure you have for the appointment.
- Your insurance card
- A photo ID
- Any forms the office may have asked you to fill out ahead of time, including a list of your current medications
- Money for your copay
- A list of questions you have for the provider
Try to arrive at least 15 minutes before your appointment. This will allow you enough time to check in. The office may have additional forms they need you to review, such as their privacy policy. Once all of that is taken care of, you will be called back to the doctor’s office to begin your appointment.
